Payment Policy:
DR. BAKER MAKES PAYMENTS AFFORDABLE, ASK US HOW.
- Private Pay:
Patients are expected to pay in full for services at each visit.
As a Thank You" for paying in full at the time of
service, you will be given a 25% discount. If you
are unable to pay your balance in full, please discuss this with
Dr. Baker prior to treatment so that special arrangements
can be made.
- Insurance:
Patients are expected to pay in full for services at each visit.
As a Thank You" for paying in full at the time of
service, you will be given a 25% discount. We
will prepare the required insurance billings so you can mail them
to your insurance company. The insurance company will then
reimburse you according to their contract with you. Patients are
financially responsible for all charges if the insurance company
should decide not to pay these bills. If you are unable to pay in
full for your services, please discuss your situation with Dr.
Baker so that special arrangements can be made.
- Managed Care:
Patients pay at the time of service their co-pay and for any
services not covered by their plan, such as supplements. We then
bill the managed care carrier to collect the remainder.
- Extended Care:
Special arrangements may be made for extended treatment protocols,
such as postural correction.
- Motor Vehicle Collision or
Personal Injury: We will make out all required insurance
billings and mail them to the insurance company. Usually your
insurance company will pay Dr. Baker directly under your PIP
policy. Depending on the circumstances of the accident, we may ask
you to pay at the time of your visit. Patients are financially
responsible for all charges if the insurance company should decide
not to pay these bills. Supplements must often be paid by the
patient.
- Workers Compensation:
We will bill the workers compensation insurance company.
They will pay Dr. Baker directly. Patients are financially
responsible for all charges if the insurance company denies the
claim. Supplements must be paid by the patient at the time they
are issued.
- Billing and Finance
Charges: Balances due in excess of thirty (30) days may
be subject to a re-billing charge of $4.00 and a finance charge of
1.5 per cent per month (18 per cent per year) unless prior
arrangements have been made.
